Medical records processing that turns discharge notes and lab results into EHR data

Medical records processing for clinics and labs. The system reads discharge summaries, referrals, lab results and external reports, extracts diagnoses, values and prescriptions and writes them to your EHR. The model runs on premise.

What hurts

Patients bring records from other providers, photos of lab results and referrals. The doctor spends part of the visit reading them and copying the key points into the chart. Less time is left for the patient.

External lab results arrive as PDFs or on paper. They cannot be trended because the EHR stores them as images. Doctors scroll through files by hand.

Sending health data to public AI services is a serious GDPR risk. So clinics either automate nothing or take a risk when staff paste texts into chatbots themselves.

What we build

  • Intake from front desk, email, scanners and the patient app
  • Recognition of printed text, lab tables and stamps
  • Extraction of diagnoses, values with units, prescriptions and dates
  • Normalisation to ICD-10, LOINC and your service codes
  • Short patient history summary for the doctor before the visit
  • EHR integration through API or HL7 FHIR
  • LLM deployment on clinic servers or a dedicated server

What you get

  • Document processing system deployed inside the clinic network
  • EHR and patient chart integration
  • Review interface for medical staff
  • Source code, documentation and data protection measures
  • Test document set with a quality report

How we measure the result

  • Doctor time spent on external records
  • Share of documents stored in the EHR as structured data
  • Value extraction accuracy on the control set
  • Corrections made by staff

Questions

How much does it cost?

From $10,000. The price depends on document types, your EHR and integration method, and whether hardware has to be installed at the clinic. We give a fixed estimate within 2–3 days after discovery.

Where is patient data processed?

Only on clinic servers or on a server dedicated to you. We use our own LLM, so data never goes to third-party models. NDA and DPA are signed before access, and we hold ISO/IEC 27001:2022.

Does the system diagnose patients?

No. It only transfers and structures what is written in the documents and drafts a summary. Clinical decisions stay with the doctor. AI sometimes makes mistakes, so every value links to its source and is reviewed.

Which EHR systems do you work with?

We connect through API, HL7 FHIR or file exchange. During discovery we check which data your EHR can accept in structured form. We also review EU AI Act and GDPR requirements for your use case.

How do you roll it out in a clinic?

In stages. First one department and one or two document types, such as discharge summaries and lab results. Staff review every transferred value while we measure accuracy on the control set. Only then do we add other departments. Everything runs inside the clinic network.
